The assessment asks for your details, the care recipient's details and your relationship to them. Provide accurate information about the circumstances and the dates you need. Check your workplace or institution's requirements for supporting documentation.

A doctor assesses your circumstances. Approval and the dates requested are not guaranteed.

6am–midnight, seven days a week (Sydney time). Delivery depends on the assessment; approved certificates arrive by email.
If your request is declined, you receive a full refund. You are welcome to make another request, which will be assessed on its own circumstances.

A work certificate request relates to your own illness. A carer's certificate request relates to caring responsibilities and asks for information about the person you are caring for.
Can I request a certificate for several days of care?
Single-day, two-to-three-day and four-or-more-day requests are available. The doctor assesses whether the requested certificate and duration are appropriate.
What happens if my request is declined?
If your request is declined, you receive a full refund. You are welcome to make another request, which will be assessed on its own circumstances.
