Documentation for your workplace absence
Choose a single day, two to three days, or four or more days to match your request. The duration you select is a request for assessment, not a guarantee of approval. Check your workplace's notification and evidence requirements before submitting your certificate.

Yes. Choose the two-to-three-day or four-or-more-day option and specify your dates. A doctor assesses the requested period based on the information you provide.
How can my employer check the certificate?
Medigate certificates include a QR code for authenticity checks. The public verification page also accepts a certificate reference number and surname.
Does a certificate automatically approve my sick leave?
A doctor assesses whether a medical certificate is appropriate. Your employer handles leave arrangements and evidence requirements. Let your workplace know about your absence using its usual process; requesting a Medigate certificate does not notify your employer.
Can I use Medigate if my workplace needs its own form?
Contact our team before paying if your workplace requires a particular form or extra information. You must be 18 or over to request an assessment through Medigate.
What happens if my request is declined?
If your request is declined, you receive a full refund. You are welcome to make another request, which will be assessed on its own circumstances.
